Casemanagers serve children and adults who have a serious and persistent mental illness which interferes with their capacity to function in significant areas of daily life over a prolonged period of time.
Blended/Intensive Casemanagement & Resource Coordination Services are non-clinical primary mental health services to insure that the most seriously mentally ill individuals receive additional support geared to improving the quality of their lives and reducing psychiatric hospitalization. Casemangers use natural occurring supports in the community, i.e., family, friends and neighbors. Only when these supports have been exhausted and the consumers needs remain unmet are generic service providers used.
Casemanagers employ a variety of individualized strategies to assist consumers in developing a more functional and qualative lifestyle consistent with the goals established by the consumer. They provide consumers with information about appropriate community services and resources and support them in accessing these services. They teach consumer basic aspects of independent living, assist in behaviors learned at treatment program and are often the first line of intervention during crises.
