Assertive Community Treatment (ACT)/Forensic Assertive Community Treatment (Fact)

    The Assertive Community Treatment Team is a voluntary community based program providing comprehensive mental health, addictions assessment, rehabilitation, and support services to persons 18 years of age or older with serious and persistent mental illness and co-occurring disorders residing in Luzerne & Wyoming Counties. The majority of the aggressive outreach approach occurs in the consumer's community through a trans-disciplinary team which includes a psychiatrist, registered nurses, mental health professionals, and specialists in the fields of chemical dependency, vocational rehabilitation, housing, and peer support.

 

* The FACT Team is specifically designed for those clients meeting the several criteria for ACT but have a forensic / criminal justice history.
